IRB Request for Proprietary NLU Data
This is the form used when faculty and staff want to use data belonging to the university for purposes of institutional assessment or personal research. Guidelines are included.

IRB Change of Status Request Form
Any change to a previously submitted protocol, such as an extension of time, the addition of investigators, a change of study method, or a change of location, requires the submission of a change of status form along with appropriate documentation. If a new protocol is required, the primary research investigator will be notified.

IRB Change of Status due to COVID-19 Form
The circumstances caused by COVID-19 may necessitate changes to already-approved research.  For example, in-person data collection may need to be done virtually instead. Any COVID-19-caused changes to an approved application require the submission of this abbreviated change of status form. This form is for COVID-19-related changes only.
